Gary Wayne Forman, 71 of Blythedale passed away Monday, June 20, 2011 at his home. Gary, the son of Walter Inskip and Lavene (Comer) Forman was born October 15, 1939 in Mineral Wells, Texas. His father preceded him in death. He was also preceded in death by a daughter, Donna Stewart Nichols. Gary served in the US Air Force during the Vietnam conflict. He was a retired iron worker and welder. Gary was a member of the Kirkley Chapel Church of Ridgeway. Survivors: Wife, Janice of the home; Son, Walt (Tina) Forman, Bethany; Daughter, Carol Wood, Ft. Worth, TX; Mother, Lavene Forman, Ft. Worth, TX; Son, Phil (Kathi) Stewart, Bethany; 4 grandchildren and 11 great-grandchildren.
